Incident Reporting During Placements
Students in the Midwifery Education Program (MEP) must report:
- any clinical incidents in which they are involved during their placements.
- any incidents which result in their own personal injury.
There are two different processes for these incidents.
1. Clinical Incident Reporting
Clinical incidents are any incidents which might reasonably result in a future professional liability claim. These include incidents leading to poor outcomes, or near misses, and those leading to significant client dissatisfaction. Students seeking greater clarity about whether or not to report a particular incident, should seek advice from their preceptor(s), and their tutor.
As soon as it is practical, students should alert their primary preceptor and their tutor of the incident, and use the (PDF file) Clinical Incident Reporting Form to report the incident to the department. Students should seek support from their preceptor and/or tutor about how to fill out the form and should send the form to their tutor for vetting prior to submission. The form should be submitted without delay to the MEP Student Affairs Team at mepadvising@torontomu.ca.
2. Personal Injury Reporting
Students should report any incidents which result in their own personal injury. This would include any injuries incurred while engaged in class or placement related activities and also injuries incurred on the way to or from clinical placement work.
All needle stick and/or bodily fluid splash incidents should be reported as personal injury incidents.
Students experiencing a personal injury incident outside of the university (e.g. while in a clinical placement) should follow any local protocols for reporting and obtaining personal medical attention. If additional medical attention is required, seek follow-up from your family practitioner or the 自淫系列 Medical Centre.
As soon as it is practical, students should alert their primary preceptor and their tutor of the personal injury incident, and use the (PDF file) Personal Injury Incident Reporting Form to report the incident to the department. The form should be submitted without delay to the MEP Student Affairs Team at mepadvising@torontomu.ca and the MEP Placement Team at midwiferyplacements@torontomu.ca. Students unclear about how to fill out the Student Incident Reporting Form should seek advice from their tutor.
Students reporting a personal injury incident, may also be required to submit Workplace Safety and Insurance Board (WSIB) reporting.
WSIB reporting is not required for injuries requiring first aid treatment only.
First aid is defined by the WSIB as a 鈥渙ne-time treatment or care and any follow-up visit(s) for observation purposes only. First aid includes, but is not limited to: cleaning minor cuts, scrapes, or scratches, treating a minor burn, applying bandages and/or dressings, applying a cold compress, cold pack, or ice bag, applying a splint, and changing a bandage or a dressing after a follow-up observation visit.鈥�
WSIB reporting is required if the injury/incident may result or has resulted in any or all of the following:
- Health care by a medical practitioner for which there is a fee for service (e.g . physician, chiropractor, dentist or hospital)
- Loss of time from work, placement and/or university beyond the date of the incident
- Exposure to and/or contraction of a communicable disease (e.g. TB exposure, needlestick injury)
WSIB Reporting
To submit Workplace Safety and Insurance Board (WSIB) reporting, contact the MEP Placement Team at midwiferyplacements@torontomu.ca for assistance in filling out these forms. The MEP Placement Team is responsible for submitting all the forms to the WSIB and for ensuring that they are filled out correctly prior to submission.
Complete and submit the following WSIB documentation:
- (PDF file) Letter of Authorization: Completed by the MEP Placement Team with information provided by the student. Requires a signature from a representative of the placement site, typically the midwife preceptor present when the injury occurred, the student's primary preceptor (e.g. OB, family physician, LC, nurse or a nurse manager).
- (PDF file) Post Secondary Student Unpaid Work Placement Workplace Insurance Claim Form: Completed by the student with contributions in Section D from a representative from 自淫系列 MEP, typically the MEP Placement Team, and a representative from the placement site, typically the midwife present at the time of the injury, the student鈥檚 primary preceptor (e.g. OB, family physician, LC, nurse or a nurse manager)

- (PDF file) Report on Needlestick Injury or Body Fluid Splash: Completed by a representative of 自淫系列, typically the MEP Placement Team, with input from the student. Note that your "employer name" for this form is the Ministry of Colleges, Universities, Research Excellence and Security (not your placement agency's name).
Note that other documentation may be requested in addition to the above noted forms by WSIB (e.g. medication prescription, physician notes, ER report, occupational health report). It is advisable to keep copies of any documentation pertaining to the incident. More detailed information may be obtained on the .
